This form is only intended for reporting safety concerns involving products for which EARP B.V. is listed as the Authorized Representative under EU product safety legislation. Your report will be reviewed and where required, forwarded to the legal manufacturer and/or national authorities in accordance with EU safety laws who will determine if further action is needed where applicable and appropriate.

Please note that reports submitted via the EU system will not be automatically shared with EARP or the product’s manufacturer. If your concern involves serious safety risk(s), we recommend submitting your report directly to EARP via this form to ensure timely coordination and action.
